• How many people attended your FPAW event?
• What kinds of professionals/organizations were involved in your FPAW event?
• What kinds of consumers were involved in your FPAW event?
• What kinds of information were disseminated to participants on fall prevention at your FPAW event?
• Did you use any FPCE materials, and if so which ones
• What kinds of products were generated for your FPAW event (e.g., community walkability audits, proclamations, press releases)?

Additional Suggestions:

• Record any anecdotes from the day (e.g., participant comments, comments from local policymakers).
