Instructions for Customizing Data Slide

The Fall Prevention Community Advocacy Toolkit PowerPoint Presentations contain a number of data slides which are customizable by county or region.

Fall-related hospitalizations and average cost of hospitalization due to a fall by county are available here.

Graphs and charts in slides 4 and 6 (and slide 9 in 10-Minute Presentation) can be customized in one of two ways:

Accessing the California Department of Public Health’s EPICenter (www.applications.dhs.ca.gov/epicdata/), which housesCalifornia injury data on-line, and manually editing the Excel-based graphs and charts contained in the presentation.

OR

Contacting the Fall Prevention Center of Excellence and requesting that graphs and charts be prepared for your county or region. Please send data requests to rachel.zerbo@cdph.ca.gov.
